CASE 1: The patient is a 30-year-old human immunodeficiency virus (HIV) positive male who presents with a three-month history of multiple crusted plaques and painful ulcers on the face.

CASE 2: A 36-year-old male single Chinese fisherman was admitted with complaint of unwellness and weakness for four days. About four weeks previously an abscess in the right lower chest wall had been treated by a general practitioner with incision, drainage and antibiotic.

CASE 6: A 49-year-old male racehorse trainer presented with an influenza-like illness six days after the death of a mare in his stables. A chest x-ray showed diffuse alveolar shadowing. He rapidly deteriorated and died seven days after admission.

CASE 7: A 35-year-old male presented with a 12-day history of sore throat, headache, drowsiness, vomiting and neck stiffness after assisting at an autopsy on a racehorse. He recovered from this, but 12 months later he had a recurrence of symptoms, became comatosed and died.
